From a Khrunichev press release

BAIKONUR COSMODROME, Aug. 18, 2008 —  The Breeze M upper stage that will be used for the next ILS Proton launch arrived here Sunday at Yubileiny Airport on an Antonov cargo plane provided by the Russian air carrier Polyot.  

Soon a team from Khrunichev State Research and Production Space Center will arrive to start preparations of the upper stage and the  Proton M vehicle for the September launch of Telesat's Nimiq 4 satellite.  The Proton vehicle and the Breeze M upper stage are built by Khrunichev of Moscow. 

ILS and Telesat signed a contract in 2006 to launch Nimiq 4 on а Proton Breeze M vehicle.  It will be Telesat's fifth launch on a Proton launch vehicle.
